June 6, 2005 – Milwaukee, WI. The Catholic Community Foundation, which began operation on July 1, 2001, funds organizations and causes that support the mission of the Catholic Church in Southeastern Wisconsin.
The Catholic Community Foundation has awarded grants to 13 Milwaukee organizations that provide health care services to the medically indigent. "We are excited to be able to assist these worthy organizations that provide such important medical services to those in need," said Stephen N. Graff, Chairman of the Board.
Archdiocese of Milwaukee - $1,840
This grant will be awarded to the Catholic Stewardship Appeal.
Adult Learning Center - St. Mary's Foundation - $10,000
The grant will assist in funding the work of a registered nurse at the Adult Learning Center that provides primary health care, health based counseling, prevention/wellness education and a connection to health care service available in the community.
Angel of Hope Clinic - The Foundation for St. Francis Hospital - $10,000
This grant will assist in supporting the expenses of the Angel of Hope Clinic, located at the Hope House shelter, which provides basic health care services to poor and homeless people on Milwaukee's near south side.
CommonGround Ministry, Inc. - $10,000
This grant helps support Elena's House, which provides physical, spiritual and emotional support to people infected and affected by HIV/AIDS.
CORE/El Centro - $10,000
This grant will provide funds for a nurse/health advocate making health care available to those who wouldn't otherwise have access. Core/El Centro is a natural healing center for underserved and low-income clients and works in collaboration with Walker's Point Community Clinic.
House of Peace Community Nursing Center - Province of St. Joseph of the Capuchin Order, Inc. - $12,500
This grant will support the House of Peace Community Nursing Center. Their work is to assist individuals in attaining an improved quality of life and to provide medically necessary services and medicine to clients who lack insurance, information, or money and therefore, lack a medical home.
Madre Angela Dental Clinic - St. Mary's Foundation - $12,500
This grant will assist in supporting the Urgent Care Program that provides emergency dental care services. Madre Angela Dental Clinic is an oral hygiene clinic dedicated to serving those who are homeless or uninsured.
Medical College of Wisconsin-Family House Assessment and Referral Clinic - $10,000
This grant request will assist in supporting the expenses of the Family House Assessment and Referral Clinic. This clinic serves a medically underserved population in Milwaukee that needs routine medical care and emergency referral services.
Milwaukee Christian Center - $8,750
Funding will be used to increase access to services for Southeast Asian seniors and facilitate more proactive and healthy behaviors. The Milwaukee Christian Center provides health education, screenings, immunizations and referrals to Milwaukee's Southeast Asian senior population.
Near South Side Catholic Schools Association - $15,000
This grant will help fund a school nurse for three schools on the near south side - St. Adalbert, Prince of Peace/Principe de Paz, and St. Rafael. The nurse works directly with the medical needs of students, assists parents in their search of medical providers, educates parents about the medical needs of their children, and trains teachers/staff to properly handle medical situations.
Older Adult Day Services Center - Catholic Charities of the Archdiocese of Milwaukee - $9,760
The grant will help fund a registered nurse on staff at the center. The nurse provides health maintenance, including blood pressure testing, supervision of medications and referral, as well as staff in-service programs.
Penfield's Children Center - $12,500
This grant will help support Penfield's Special Care Nursery, in providing nursing services, physical, occupational and speech therapies, special education, and childcare for mentally fragile and at-risk children from birth to age three.
Rosalie Manor Family and Community Services - $12,500
This grant will assist two home visiting nurses in providing basic health and medical services for the Family Program. Rosalie Manor Family and Community Services is committed to working for the prevention of child abuse, child neglect and teen pregnancy.
St. Ben's Clinic at Cathedral Center - St. Mary's Foundation -$12,500
This grant will assist a nurse practitioner who provides medical services at St. Ben's Clinic at the Cathedral Center. This outreach service helps to provide medical services for homeless women and children.
S.E.T. Ministry, Inc. - $12,000
This grant will support two nurse case managers who will assist in providing for the health care needs of 40 at-risk families through the Family Health Promotion Project. S.E.T. Ministry, Inc. has been successful in implementing programs that serve the low income, underserved and overlooked people in Milwaukee.
